Community Care Lead – Aged Care Registered Nurse
$115,000  Super Salary Packaging • Monday – Friday • Community Aged Care

We’re partnering with a highly regarded not-for-profit aged care provider to recruit a Care Lead - a senior clinical role that gives you leadership responsibility across community aged care services, with autonomy, variety and impact every day.

What You’ll Do In this lead clinician role you will:

• Provide clinical leadership and oversight across community aged care services
• Conduct in-home clinical assessments, care planning, review and evaluation
• Support and mentor community care staff with clinical guidance
• Be the clinical escalation point for complex care needs
• Work closely with families, GPs and allied health professionals
• Ensure clinical governance and compliance with Aged Care Quality Standards
• Maintain accurate and audit-ready clinical documentation

• Current AHPRA Registered Nurse registration
• Strong clinical capability with experience in Support at Home / community aged care
• Prior experience in leadership
• Excellent communication, clinical assessment and care planning skills
• Confidence working autonomously across community settings

• $115,000 super salary packaging — rewarding and competitive
