As a Care Manager, you’ll play a pivotal role in supporting older Australians to live safely, independently, and with dignity in their own homes. You’ll combine clinical expertise with care coordination, working closely with clients, families, and service providers to deliver tailored, wellness-focused care.

🧠 What You’ll Be Doing

• Conduct comprehensive assessments and develop personalised care plans
• Coordinate in-home services aligned to client goals and preferences
• Provide clinical oversight and ongoing monitoring of care delivery
• Build trusted relationships with clients, families, and stakeholders
• Advocate for choice, independence, and quality outcomes
• Maintain accurate documentation and support compliance requirements

• AHPRA-registered nurse
• Background in aged care, community, or care coordination roles
• Strong communication and relationship-building skills
• Organised, proactive, and confident working autonomously
• Passionate about wellness, reablement, and person-centred care
• Ideally 2 years experience in community care
