An exciting opportunity has become available for an experienced and resident-focused Care Manager to join a growing residential aged care provider that's redefining how care is delivered across its network of communities.

As the Care Manager, you'll be responsible for leading the clinical and care operations of the home, ensuring exceptional outcomes for residents while developing and supporting the nursing and care team.
Working closely with the Facility Manager, you'll oversee care planning, clinical risk management, medication safety and quality and compliance initiatives, fostering a positive, high-performing culture that reflects the organisation's holistic, person-centred approach to care.
Key Responsibilities
- Lead and support the nursing and care team to deliver high-quality, person-centred care.
- Oversee care planning, assessment schedules and clinical documentation across the home.
- Monitor and respond to clinical risk, including deterioration, falls, wounds and behavioural changes.
- Ensure safe and appropriate medication management practices, including psychotropic and restrictive practices requirements.
- Drive compliance with the Aged Care Quality Standards and other regulatory requirements.
- Support the admissions process and coordinate care needs for new residents.
- Build strong relationships with residents, families, GPs and allied health providers.
- Mentor, educate and develop clinical staff to build team capability.
- Current AHPRA registration as a Registered Nurse (essential).
- Strong experience as a Care Manager, Clinical Care Coordinator, Clinical Nurse Manager or similar clinical leadership role within residential aged care.
- Strong knowledge of care planning, clinical risk management and the Aged Care Quality Standards.
- Confident leading and developing a multidisciplinary nursing and care team.
- Sound understanding of medication management, including psychotropic and restrictive practices requirements.
- Passionate about person-centred, high-quality care and continuous improvement.
- Excellent communication, documentation and stakeholder management skills.
