Description

My client a well-established aged care provider are looking for an experienced Care Manager for a large, fully accredited residential home located on the NSW Mid North Coast, just a short drive from Port Macquarie.
 
This is a full-time leadership opportunity suited to a strong clinical leader who enjoys being hands-on, driving quality outcomes and supporting team performance.
 
About the Role
Reporting directly to the Facility Manager, the Care Manager will lead and coordinate the clinical, care and allied health teams to ensure high standards of resident-centred care. You will play a key role in maintaining compliance, strengthening clinical governance and ensuring funding optimisation through accurate AN-ACC processes.
 
Key Responsibilities

  • Oversee the day-to-day clinical and care operations of the facility
    • Lead and support Registered Nurses and care staff to deliver high-quality, evidence-based care
    • Coordinate roster planning to ensure appropriate staffing levels and skill mix
    • Ensure timely and accurate AN-ACC assessments to maximise funding
    • Manage clinical incidents, investigations and continuous improvement processes
    • Oversee clinical assessments and ensure care plans are current and reflective of residents’ needs
    • Maintain compliance with the Aged Care Act and Quality Standards
    • Promote a positive, team-focused culture across the home
 
About You
  • Registered Nurse with current AHPRA registration
    • Demonstrated clinical leadership experience within residential aged care
    • Strong knowledge of Aged Care Quality Standards and AN-ACC
    • Confident in staff supervision, mentoring and performance management
    • Excellent communication skills with the ability to build strong relationships with residents and families
    • Organised, solutions-focused and committed to continuous improvement
