An exciting opportunity is available for an experienced Care Manager to join a well-established not-for-profit aged care provider at a large residential aged care facility in South Grafton.
This role is ideal for a strong clinical leader who is passionate about delivering high-quality, person-centred care and supporting teams to thrive in a values-driven environment.

About the Role Reporting to the Facility Manager, you will play a key leadership role in overseeing clinical care delivery, supporting nursing teams, and ensuring compliance with the Aged Care Quality Standards. You will work closely with residents, families, and multidisciplinary teams to maintain excellent clinical outcomes and a positive care culture.

Key Responsibilities

• Lead and support the clinical care team, including RNs and care staff
• Ensure high standards of clinical care, compliance, and governance
• Support accreditation readiness, audits, and continuous improvement
• Build strong relationships with residents, families, and stakeholders
• Mentor and develop staff to deliver safe, person-centred care

• Current AHPRA registration as a Registered Nurse
• Previous experience as a Care Manager, Clinical Manager, or Senior RN
• Strong knowledge of the Aged Care Quality Standards and clinical governance
• Confident leader with excellent communication and people management skills
• Passion for working in a not-for-profit, values-based organisation
