We are seeking an experienced Care Manager to join a well-established team near Thornleigh, overseeing a fully accredited 100-bed nursing home. Join a respected not-for-profit provider known for delivering high-quality, person-centred care. In this role, you will work closely with the Facility Manager to oversee all aspects of clinical care, drive continuous improvement, and support your team to deliver exceptional outcomes for residents.

• Collaborative team structure: FM > 2 CMs > RNs > Care Staff
• Hands-on, supportive leadership with strong clinical governance

• Lead and manage the clinical care team to ensure excellence in care delivery
• Support compliance with the Aged Care Quality Standards
• Mentor and develop clinical staff, including RNs and Care Staff
• Collaborate with families and external stakeholders to support resident wellbeing
• Manage care plans, assessments, and risk mitigation strategies

• AHPRA Registered Nurse with strong clinical leadership experience
• Previous experience in a Care Manager or senior clinical role
• Excellent understanding of Aged Care Quality Standards and accreditation requirements
• Compassionate, hands-on leadership style with a focus on team development
• Committed to delivering high-quality, person-centred care
