A well-established residential aged care home in Portland is seeking an experienced and clinically capable Care Manager to join the team during an exciting period of growth and service enhancement.

About the Role
As the clinical lead for the service, you will oversee care delivery, support clinical governance, and mentor a multidisciplinary team to achieve strong resident outcomes.
This role combines clinical leadership, team development, compliance oversight, and quality improvement within a supportive and culturally diverse environment.
You will work closely with operational leadership to ensure best-practice care standards are maintained while fostering a positive and inclusive workplace culture.
Key Responsibilities
- Lead the clinical team to deliver high-quality resident-centred care
- Oversee care planning, assessments, and clinical documentation standards
- Support and mentor nursing and care staff to build capability and confidence
- Drive quality, compliance, and continuous improvement initiatives
- Manage clinical incidents, escalations, and complex care requirements
- Collaborate with residents, families, and allied health professionals to improve outcomes
- Monitor accreditation readiness and ensure alignment with aged care standards
- Build a positive and inclusive team culture across a multicultural workforce
You are a compassionate, organised, and highly collaborative individual who genuinely enjoys supporting residents and leading teams to deliver exceptional care.
You will also bring:
- Previous experience as a Care Manager, Clinical Manager, or senior clinical leader within aged care
- Current AHPRA registration as a Registered Nurse
- Confidence handling clinical escalations, family concerns, and compliance requirements
- Strong understanding of aged care clinical governance and quality standards
- Experience leading and developing multidisciplinary teams
- Previous experience within residential aged care
