Clinical Care Coordinator | Relocation To Adelaide!

A fantastic opportunity has become available for an experienced Registered Nurse to join a well-established not-for-profit aged care provider as a Clinical Care Coordinator in Adelaide.

This is more than a Clinical Care Coordinator role - it's an opportunity to take the next step in your leadership career while joining an organisation genuinely committed to delivering exceptional, person-centred care. Working alongside an experienced and supportive leadership team, you'll play a key role in the day-to-day clinical operations of the home, mentoring staff, strengthening clinical practice and ensuring residents receive safe, high-quality care.

The Role
As the Clinical Care Coordinator, you'll provide hands-on clinical leadership across the home, supporting the delivery of safe, compliant and person-centred care. Working closely with the Facility Manager and broader leadership team, you'll oversee clinical governance, mentor staff and foster a culture of accountability, quality and continuous improvement.

Key responsibilities include:

  • Providing day-to-day clinical leadership and support across the home
  • Mentoring, coaching and developing Registered Nurses, Enrolled Nurses and Personal Care Workers
  • Overseeing care planning, clinical documentation and resident assessments to ensure compliance with the Aged Care Quality Standards
  • Supporting accreditation readiness, internal audits and continuous improvement initiatives
  • Monitoring clinical incidents, risk management processes and quality indicators
  • Building strong relationships with residents, families and multidisciplinary teams to ensure exceptional care delivery
  • Leading by example and creating a positive, supportive and collaborative team culture
About You
You're a clinically confident Registered Nurse who enjoys supporting others and making a genuine difference to resident outcomes. You lead with empathy, communicate effectively and are passionate about developing people while delivering exceptional clinical care.

To be successful, you'll have:
  • Current AHPRA registration as a Registered Nurse
  • Previous experience as a Clinical Care Coordinator, Clinical Lead, Senior Registered Nurse or similar leadership role within residential aged care
  • Strong understanding of the Aged Care Quality Standards, clinical governance and accreditation requirements
  • Experience mentoring and developing nursing teams
  • Excellent communication, organisational and relationship-building skills
  • A proactive, hands-on leadership style with the confidence to support and influence others
