An exciting opportunity has become available for an experienced and passionate Clinical Care Coordinator to step into a a respected not-for-profit residential aged care community located in Brunswick.
This is an opportunity to join a close-knit home where relationships, cultural understanding and person-centred care are at the heart of everything they do. You will play a key role in supporting residents, families and care teams, ensuring high-quality clinical outcomes while helping drive a positive and supportive environment for both residents and staff.
Reporting directly to an experienced General Manager with a strong clinical background, you'll be supported by a leader who understands the complexities of clinical care and is passionate about mentoring and developing their team. Report directly to an experienced clinically qualified General Manager who provides hands-on mentorship and support.
The Role
As the Clinical Care Coordinator, you'll work closely with the General Manager to provide hands-on clinical leadership and ensure residents receive safe, high-quality, person-centred care.
You'll support the clinical team, oversee day-to-day clinical operations and help drive quality, compliance and continuous improvement across the home.
Key responsibilities include:
- Providing leadership and support to Registered Nurses, Enrolled Nurses and Personal Care Assistants
- Overseeing clinical assessments, care planning and documentation
- Supporting clinical governance, accreditation and continuous improvement initiatives
- Monitoring incidents, clinical risks and quality outcomes
- Building strong relationships with residents, families and external health professionals
- Mentoring and developing the clinical team
- Current AHPRA registration as a Registered Nurse
- Experience within residential aged care
- Previous experience as a Clinical Care Coordinator, Care Manager or senior clinical leader
- Strong understanding of the Aged Care Quality Standards and clinical governance
- Excellent communication and leadership skills
- A compassionate, resident-focused approach to care
- Experience working with culturally diverse communities will be highly regarded
- Bilingual candidates who speak Mandarin or Cantonese are encouraged to apply, although this is not essential.
