Are you an experienced aged care clinician looking to join a provider where quality care, strong relationships, and community values are at the heart of everything they do?
We are partnering with a well-established not-for-profit aged care provider seeking a Clinical Care Coordinator to join their welcoming home in Cheltenham. Working alongside an experienced Residential Manager and leadership team, you will play a key role in supporting the delivery of high-quality, person-centred care while providing leadership and guidance to the clinical team.
Key Responsibilities
- Provide day-to-day clinical leadership and support to nursing and care teams
- Oversee resident assessments, care planning, documentation, and clinical reviews
- Support the delivery of safe, high-quality, person-centred care across the home
- Monitor clinical indicators and assist with quality improvement initiatives
- Ensure compliance with aged care legislation, standards, and organisational policies
- Assist with accreditation preparation and ongoing clinical governance activities
- Support incident management, risk identification, and corrective actions
- Mentor, coach, and develop clinical and care staff to promote best practice
- Build positive relationships with residents, families, general practitioners, and allied health professionals
- Work collaboratively with the leadership team to continuously improve resident outcomes and service quality
- Current AHPRA registration as a Registered Nurse (Div 1)
- Proven experience in aged care clinical leadership, coordination, or senior nursing roles
- Strong knowledge of Aged Care Quality Standards and clinical governance frameworks
- Demonstrated ability to lead, mentor, and support multidisciplinary care teams
- Experience in care planning, assessments, and clinical documentation systems
- Sound understanding of incident management, infection control, and risk management
- Strong communication and interpersonal skills with a resident-focused approach
- Ability to manage competing priorities in a fast-paced clinical environment
- Experience working with accreditation processes and quality improvement initiatives (desirable)
- $132,000 Super Salary Packaging
