Description

Clinical Care Coordinator

  • This is a BRAND-NEW position, put into place for the home to gain more structure and support and it’s an opportunity for an experience CCC or RNIC looking to step up into a leadership role.

Summary:
An exciting opportunity exists for a compassionate and experienced Clinical Care Coordinator to join a values-driven aged care provider. In this pivotal role, you will work closely with the Facility Manager and wider clinical team to ensure the delivery of high-quality, person-centred care to residents. This is an ideal opportunity for an experienced Clinical Coordinator or Clinical Manager ready for a fresh challenge in a supportive environment.

Responsibilities
  • Provide supportive clinical leadership and guidance to nursing and care staff.
  • Champion a resident-focused culture, ensuring individual needs, preferences, and life history are respected.
  • Oversee and monitor clinical care delivery to ensure safe, high-quality outcomes for residents.
  • Delegate tasks appropriately, considering the skills, scope, and capability of team members.
  • Use critical thinking and evidence-based practice to support decision-making and continuous improvement.
  • Act as the point of contact or deputy when senior clinical leaders are unavailable.
Requirements
  • Registered Nurse with current AHPRA registration (or eligibility).
  • Previous clinical experience in aged care, acute, sub-acute, community, or similar care environments.
  • Understanding of the care needs of older people and a commitment to person-centred practice.
  • Strong communication skills with the ability to build positive relationships with residents, families, and colleagues.
  • Ability to work independently and as part of a multidisciplinary team.
  • Good organisational and time management skills, with a proactive and flexible approach.
  • Basic to intermediate computer literacy, with confidence using clinical systems or the willingness to learn.
