Clinical Care Coordinator – Nambucca Heads
The Opportunity
A well-established residential aged care provider in Nambucca Heads is seeking an experienced and clinically capable Clinical Care Coordinator to join their team on the beautiful NSW Mid North Coast.

About the Role
As the Clinical Care Coordinator, you will play a key role in supporting the delivery of safe, effective and person-centred clinical care across the home.
This full-time position combines clinical leadership, hands-on nursing care, care planning, mentoring and staff support, while working closely with residents, families, healthcare professionals and the leadership team.
You will have the opportunity to lead, mentor and support the care team, contribute to positive resident outcomes and help maintain high standards of clinical practice within the home.
The role is particularly suited to an experienced Registered Nurse who is ready to step up, take on greater responsibility and have their clinical expertise recognised.
Key Responsibilities
- Coordinate the admission of new residents, including comprehensive clinical assessments and development of person-centred care plans
- Lead, mentor and support the care team to deliver safe, high-quality clinical care
- Provide hands-on nursing care, including medication administration, wound management and ongoing clinical monitoring
- Maintain accurate and comprehensive clinical documentation
- Oversee infection prevention and control practices
- Ensure compliance with relevant regulatory standards and accreditation requirements
- Communicate effectively with residents, families and external healthcare providers
- Collaborate with multidisciplinary care teams to achieve positive resident outcomes
- Contribute to quality improvement initiatives and ongoing clinical excellence
- Support a culture of teamwork, genuine connection and person-centred care
You are a skilled, dedicated and clinically capable Registered Nurse who is ready to take the next step in your aged care career.
You will also bring:
- Bachelor of Nursing with current AHPRA registration
- Minimum 2 years' nursing experience in residential aged care
- Experience in a similar clinical care role will be highly regarded
- Strong clinical assessment, documentation and evidence-based practice skills
- Demonstrated ability to work collaboratively within a multidisciplinary care team
- Excellent communication and interpersonal skills
- Confidence engaging with residents, families, colleagues and external healthcare professionals
- Proficient computer and information management skills, including Microsoft applications and clinical software systems
- Sound understanding of the aged care accreditation process
- Ability to meet relevant Aged Care Worker Screening requirements
- A genuine commitment to delivering compassionate, safe and person-centred care
