Clinical Care Coordinator
The Opportunity
We're looking for a clinically strong and motivated Registered Nurse to step into a newly created Clinical Care Coordinator position with an established not-for-profit aged care provider.
This is a great opportunity for someone who enjoys being hands-on clinically while also supporting and developing others. You'll play an important role in maintaining high standards of care, mentoring the clinical team and helping drive positive outcomes for residents.
Ideally, you'll bring around three years of residential aged care experience. However, we're equally interested in speaking with motivated RNs who may have less experience but demonstrate strong clinical skills, initiative and a genuine drive to progress their career.

- Support clinical assessments, care planning and reviews
- Monitor resident outcomes and identify areas for improvement
- Mentor staff and promote high standards of clinical practice
- Support incident management and clinical follow-up
- Assist with maintaining accurate and compliant documentation
- Contribute to quality and continuous improvement initiatives
- Promote safe, person-centred care across the team
- Current unrestricted AHPRA registration as a Registered Nurse
- Residential aged care experience, ideally around 3 years
- Strong clinical assessment and decision-making skills
- Leadership experience advantageous but not essential
- Confident, proactive and willing to take initiative
- Strong communication and relationship-building skills
- Genuine ambition to develop within clinical leadership
