About the Role
An exciting opportunity is available for an experienced Care Manager to join a large, well-established residential aged care facility in Alexandra Hills. Working in close partnership with the Residential Aged Care Manager, you will play a critical leadership role in delivering exceptional clinical care and ensuring strong clinical performance across the home.
This role is central to supporting residents and their families as they transition into aged care, ensuring a compassionate, personalised, and holistic approach to care from day one.
Key Responsibilities
- Lead the delivery of high-quality clinical and holistic care to all residents
- Partner with the Residential Aged Care Manager to drive clinical governance and performance outcomes
- Conduct comprehensive resident assessments, care planning, implementation, and evaluation
- Build meaningful relationships with residents and families, supporting them through the transition into care
- Provide leadership, mentorship, and supervision to nursing and care teams
- Oversee and collaborate with allied health professionals and medical practitioners within the home
- Ensure compliance with all aged care standards, accreditation requirements, and regulatory frameworks
- Drive a culture of continuous improvement, safety, and clinical excellence
- Manage competing priorities in a dynamic environment, ensuring high standards are consistently met
About You
- Registered Nurse with current AHPRA registration (essential)
- Proven experience in a senior clinical leadership role within aged care
- Strong understanding of aged care accreditation standards and clinical governance
- Demonstrated ability to lead, mentor, and develop high-performing teams
- Exceptional communication and interpersonal skills, with the ability to engage effectively with residents, families, and stakeholders
- Highly organised with the ability to multitask and problem solve in a fast-paced environment
- Strong leadership presence, influencing skills, and commercial awareness
- Passionate about delivering person-centred care and improving resident outcomes
