Clinical Care Manager | Clyde North 

Lead a high-performing clinical team and make a genuine difference in aged care

An excellent opportunity has become available for an experienced Registered Nurse to step into a Care Manager position within a well-established residential aged care home in Clyde North.

This is a highly operational leadership role suited to a clinically confident RN who enjoys being present on the floor, supporting their team, building relationships with residents and families, and driving strong clinical outcomes. You will work closely with the Facility Manager and wider leadership team to ensure safe, high-quality, resident-centred care is delivered every day.

With a supportive culture, strong systems in place and the opportunity to lead a committed care team, this role is ideal for a Care Manager looking to take ownership of clinical operations and continue developing their leadership career.

The Role
As Care Manager, you will be responsible for leading the day-to-day clinical operations of the home and ensuring residents receive safe, personalised and high-quality care.
Key responsibilities include:

  • Leading, mentoring and supporting Registered Nurses, Enrolled Nurses and Personal Care Assistants
  • Overseeing clinical care delivery, assessments, care plans and documentation
  • Managing medication, wound care, infection prevention, falls management and clinical risk
  • Supporting the Facility Manager with staffing, rostering, team performance and day-to-day operations
  • Building strong relationships with residents, families, GPs, allied health professionals and external stakeholders
  • Monitoring clinical indicators and contributing to continuous improvement initiatives
  • Supporting compliance with the strengthened Aged Care Quality Standards
  • Assisting with audits, accreditation preparation, incident management and clinical governance
  • Creating a positive, accountable and supportive workplace culture
About You
We are looking for a hands-on and people-focused Care Manager who can confidently lead a team while maintaining a strong clinical presence throughout the home.

To be successful, you will have:
  • Current AHPRA registration as a Registered Nurse
  • Previous experience in a Care Manager, Clinical Manager, Clinical Care Coordinator or senior RN role within residential aged care
  • Strong knowledge of clinical governance, quality standards and aged care compliance
  • Demonstrated experience leading, mentoring and supporting care teams
  • Confidence managing clinical risk, documentation, incident reporting and care planning
  • Excellent communication skills and the ability to build trust with residents, families and staff
  • A proactive, organised and approachable leadership style
  • A genuine passion for delivering high-quality, resident-centred care
