Clinical Care Partner | Community Aged Care
A wonderful opportunity has become available for a compassionate and experienced Registered Nurse to step into a Clinical Care Partner Team Lead role with a deeply values-driven, community-focused aged care organisation with a rich history.

The Role
As Clinical Care Partner Team Lead, you will provide operational leadership and clinical oversight to a pod of Care Partners delivering in-home and community aged care services across funding streams including Support at Home, CHSP and private services. You will be the go-to person for your team - guiding clinical practice, supporting quality outcomes and ensuring care is always aligned with the Aged Care Quality Standards.
Key responsibilities include:
- Leading, mentoring and supporting a pod of Care Partners in their day-to-day practice
- Overseeing the clinical quality of in-home and community aged care service delivery
- Supporting your team to deliver culturally sensitive, trauma-informed and person-centred care
- Managing incidents and complaints with professionalism, care and a focus on continuous improvement
- Ensuring alignment with the Aged Care Quality Standards across your team and client base
- Monitoring changing client needs and ensuring timely escalation and referral pathways are in place
- Collaborating closely with two other Operational Leads and reporting into the broader leadership team
- Being a visible, approachable and positive presence for both your team and the clients they support
You are a warm, organised and clinically confident Registered Nurse who understands what it means to support someone to live independently at home. You lead with empathy, bring people together naturally and approach your work with genuine humility - understanding that for many clients, allowing someone into their home and their care is an act of trust that must be earned and respected.
To be successful, you will have:
- Current AHPRA registration as a Registered Nurse
- Experience working in a community aged care setting - this is essential
- Some experience in a leadership, coordination or senior clinical role
- A solid understanding of the Aged Care Quality Standards and compliance requirements
- Confidence managing incidents and complaints in a constructive and person-centred way
- Strong communication skills and a natural ability to build trust with your team and clients
- A genuine passion for community aged care and supporting people to remain at home
