Description
We are currently working with a well-established and highly regarded healthcare provider, to find an experienced and compassionate Care Partner to join their Home Care Services team.

In this role, you will work closely with clients to support them in maintaining their independence, achieving their goals, and continuing to live safely and comfortably in their own homes.
Key Responsibilities
- Develop individualised care plans in collaboration with clients, based on their assessed needs, goals, and preferences
• Conduct ongoing assessments and adjust care plans in response to changing client needs
• Complete home and personal risk assessments while supporting dignity of risk principles
• Promote wellness and reablement approaches focused on empowering clients and maintaining independence
• Support clients with advance care planning where required
• Work within a strong clinical governance framework and ensure appropriate oversight for complex clients
• Manage client caseloads, budgets, and roster coordination effectively
• Build strong relationships with clients, families, and multidisciplinary teams
Essential Criteria
- University qualification in a relevant health discipline such as Nursing, Physiotherapy, Social Work, or similar
• Previous experience within aged care, community care, or a clinical environment
• Strong understanding of person-centred care principles
• Excellent communication and relationship-building skills
• Strong organisational and time management abilities
• Comfortable using computer systems and care management platforms
• Current driver’s licence and access to a fully insured vehicle
