A large residential aged care facility in the Hunter valley Region is seeking an experienced and clinically capable Nurse Practitioner to join the team.
This is a highly rewarding opportunity for a confident, resident-focused clinician who enjoys working autonomously while remaining part of a collaborative multidisciplinary team. You will play a key role in improving resident outcomes, reducing unnecessary hospital admissions and delivering high-quality, person-centred care.

About the Role
As the clinical lead for advanced resident assessment and management, you will provide comprehensive clinical care while working closely with GPs, nursing teams and allied health professionals.
This role combines advanced clinical assessment, prescribing, diagnostics, care planning, education and mentoring within a well-supported residential aged care environment.
You will have the opportunity to work at the top of your scope while helping shape a proactive model of care focused on resident outcomes, quality of life and hospital avoidance.
Key Responsibilities
- Conduct advanced clinical assessments and develop treatment plans for residents
- Prescribe medications and order diagnostic investigations within scope of practice
- Develop shared-care partnerships with GPs and contribute to multidisciplinary care planning
- Deliver pharmacological and non-pharmacological management strategies
- Support hospital avoidance initiatives through proactive clinical intervention
- Provide education, mentoring and clinical supervision to nursing teams
- Develop and contribute to aged care clinical guidelines and best-practice initiatives
- Communicate effectively with residents, families and healthcare professionals regarding diagnosis and treatment options
About You
You are a resilient, professional and highly capable clinician who enjoys working collaboratively to improve resident outcomes.
You will also bring:
- Current registration as a Nurse Practitioner with AHPRA
- Ability to autonomously assess, investigate and implement treatment within scope of practice
- Experience working within aged care
- Confidence managing complex clinical presentations
- Experience collaborating with GPs and multidisciplinary teams
- Current National Police Check and ability to meet vaccination requirements
